USCIS Medical Exam (Form I-693)

The USCIS Form I-693 medical exam is required for adjustment of status (green card) applicants. The exam is performed by a USCIS-designated civil surgeon and includes a physical exam, lab work, review of medical history, and completion of the required documentation. You’ll need your medical records, immunization history, and any relevant documentation.

N‑648 Medical Waiver Exam for U.S. Citizenship

Form N-648 is a medical certification used to request a waiver of the English and civics testing requirements for U.S. naturalization, in cases of physical or developmental disability or mental impairment. We evaluate whether your disability or condition justifies the waiver. Getting approved for this waiver is very difficult. USCIS denies most requests, even when the case appears strong. Filing this form does not guarantee approval. You’ll need your green card, medical records, doctor or therapist contact information, and, if needed, a translator.

USCIS Hardship Support Exam

This medical exam is for individuals applying for a United States visa or green card, for United States citizenship, or for certain immigration benefits when USCIS or your immigration lawyer requests that you obtain a hardship letter. This evaluation documents how your medical or psychological conditions may contribute to hardship to you or to qualifying United States relatives. It strengthens your case to integrate medical and psychiatric evidence into your hardship presentation.
