Appointments
- 📌 You must make an appointment to receive services.
- 📞 Appointments can be booked directly by calling 415-409-3456.
- 💻 Online appointments are requests only. The time is not guaranteed until staff confirms via phone.
- 💳 A credit card is required to hold an appointment and may be charged for no-shows or late cancellations.
- ⏳ Cancel or reschedule at least 72 hours in advance for N-648 appointments, or 48 hours for all others to avoid a $100 fee.
- ❌ Failure to show for an appointment will result in a $100 fee.
- 🕒 Please arrive on time for your appointment.
- ⚠️ Arrivals more than 10 minutes late will not be seen; no-show fees may apply.
- 📝 Arrive a few minutes early to complete any necessary paperwork.
- 🚫 Recharge Medical may refuse to schedule appointments or provide service for any reason.
Payment and billing
- 💵 Payment is due at the time of service.
- The patient or guardian is responsible for all charges, regardless of insurance coverage.
- 🏥 Recharge Medical bills only services performed in-office; outside services are billed separately.
- 🚫 Recharge Medical does not bill insurance or provide insurance codes.
- 💳 Credit cards are charged for services rendered.
- 🔒 Relevant info may be released to secure payment.
- ⚠️ Prices may change at any time.
- 📅 Accounts unpaid after 30 days are considered delinquent and may go to collections, including late fees and legal costs.
- 💳 Accepted payments: Visa, Mastercard, American Express, debit cards, and cash (USD).

Prices
Prices may change due to market fluctuations. Our website lists current prices. Questions? We’re happy to help you understand any changes.
Refunds
- ❌ All service and treatment charges are non-refundable.
- 💊 Package treatments: if not completed, refunds are the original package price minus the sessions received.
- 🎁 No refunds or exchanges on products, except where required by law.
Communication
- 📧 Recharge Medical or partners (e.g., Practice Fusion) may contact you via email, text, or voicemail about appointments, billing, or outstanding requirements.
- ☎️ Outside office visits, communicate with your physician via medical assistants. Direct physician visits require an appointment and may incur charges.
Prescriptions, orders, and paperwork
- 💊 All prescriptions, refills, referrals, test orders, and paperwork are issued only during appointments.
- ⏰ Requests must be made at least 7 business days in advance.
- ⚠️ Additional charges may apply.
Controlled substances
- ❌ Prescriptions for controlled substances are not provided during a first visit.
- ⚕️ Ongoing prescriptions will require referral to the appropriate specialist.
Pets
- 🐕 Only certified animals (e.g., service dogs) are allowed in the clinic.
- 🚫 Emotional support or other animals are not allowed.
Patients are responsible for
- 📞 Providing current phone, email, and mailing addresses.
- 💊 Updating any health, medication, allergy, or treatment info.
- 🏥 Ensuring Recharge Medical receives all required info (tests, vaccines, ID) for timely completion of exams, treatments, and documents.
- 💳 Informing Recharge Medical of payment info changes.
Feedback and concerns
- 💬 We want to hear about any concerns with your care. Staff usually can resolve issues quickly.
- ⚖️ For concerns about employee conduct, HIPAA, or regulations, contact the office and ask for the chief operating officer.
